Important
Not medical advice
Tell Me Again is a memory aid. It helps you remember and understand what was said in your appointment. It isn’t a medical record, it doesn’t give medical advice, and it doesn’t replace your healthcare team.
If you feel unwell or you are worried, ask a pharmacist or contact your GP. For urgent help, call NHS 111 or use 111.nhs.uk or the NHS App. In an emergency, call 999. Don’t wait for a summary if you feel unwell.
What it is for
Tell Me Again writes down what was said and explains it in plain English. It only uses what was said in the appointment. It doesn’t diagnose, suggest treatment or add medical information of its own.
AI can make mistakes
The words and the summary are made by AI from a phone recording. They can be wrong. For example, it might:
- mishear a medicine’s name, a dose or a date
- miss a word like “not”, which changes the meaning
- mix up who said what
- leave out something that was said quietly or over background noise
Read the words before you make a summary and correct anything that looks wrong. Check anything important, especially medicines, doses and dates, with your healthcare team, and follow what they tell you and what’s in your letters.
Recording other people
It’s legal to record your own appointment for your own use, but always tell the person you’re seeing before you start. Don’t share a recording or transcript that includes other people without their agreement.
